H11A DDS Annual Salary $55,874.34 + Full State Employee Benefits
Travel is extensive. Must possess a valid driver's license and maintain required car insurance.
Preferred Minimum Qualifications
- A master’s degree and three years of professional experience in social work, child support, adult protective services, child welfare, early childhood development and education, guidance and counseling, nursing, intellectual disability, probation and parole, public administration, community-based prevention, and diversionary youth services programs, juvenile justice, rehabilitation counseling, vocational evaluation or vocational rehabilitation;
- OR an equivalent combination and experience, substituting 1 additional year of qualifying experience for each year of the required education.
*Professional experience in the fields listed must have been the primary job responsibility to be considered as qualifying. Incidental performance of professional work in any area(s) shall not be considered.
Job Responsibilities:
The Programs Field Representative (PFR) will coordinate placement for child welfare cases, working with Child Welfare Staff to coordinate placement for these individuals approved for DDS services. The PFR will serve as a liaison with Child Welfare and work closely with the DDS Program Manager and PFR’s that work with custody kids. The PFR will be responsible for finding placement for children aging out of Child Welfare Level D Group Home or Sheild. The PFR is involved with Assisting the case manager with the transition meeting and ensuring all needed services are in place before the move date. Will follow the case up to 90 days by attending the 30-, 60-, and 90-day meetings if applicable. The PFR will attend the monthly State Office staffing on CW children, monthly meeting for 3 Level D homes and the 17 year old staffing monthly.
The PFR will be responsible for coordinating all ERS placements with the residential unit PFR and lead the team in all transition processes once a placement is found. The PFR will attend the monthly meeting for ERS.
The PFR oversees transitions and serves as liaison with state and area staff in the development and completion of transitions in the area, resolves transition issues as they occur, trains staff on the transition process, OKSA, and Community Transition Funds, while assisting to improve the Transition process. The PFR will assist Case Managers in resolving complex consumer service issues.
The PFR is responsible for training the transition process in the Case Manager Academy. The Program Field Representative will provide examples and instructions on the transition process. Provide tools and contact information for the Transition Coordinators.
