Programs Field Representative
H11A DDS Annual Salary $55,874.34 + Full State Employee Benefits
Travel is occasional. Must possess a valid driver's license and maintain required car insurance.
Preferred Minimum Qualifications
- A master’s degree and three years of professional experience in social work, child support, adult protective services, child welfare, early childhood development and education, guidance and counseling, nursing, intellectual disability, probation and parole, public administration, community-based prevention, and diversionary youth services programs, juvenile justice, rehabilitation counseling, vocational evaluation or vocational rehabilitation;
- OR an equivalent combination and experience, substituting 1 additional year of qualifying experience for each year of the required education.
*Professional experience in the fields listed must have been the primary job responsibility to be considered as qualifying. Incidental performance of professional work in any area(s) shall not be considered.
Job Responsibilities:
- Facilitate, coordinate, plan, and monitor the placement and programming for service recipients served through the adult DDS alternative group homes (AGH) for 96 individuals.
- Approve or deny placement ready plans for individuals served through the adult DDS AGH program.
- Conduct monthly (i.e. at least one per month) on-site AGH home reviews to ensure consistent implementation of DDS rules and policies.
- Maintain a tracking system to track issues and trends to assist in the remediation and early prevention of problems. This includes review of the Client Contact Manger system, use of a data base, and site visits when necessary.
- Develop improved accountability measures, prevention, or resolve difficult problems.
- Provide technical assistance to DDS and provider agency staff consistent with DDS rules and procedures.
- Participate in team meeting, when requested, to provide technical assistance in remediation of issues or concerns.
- Responsible to create and provide AGH case management and provider training. Provide technical assistance, training to improve quality services, early identification and problem remediation.
- With supervision, direct field staff in the placement planning for criminal cases referred to DDS by the Court system and the Office of Public Guardian. Assist in coordination with non-waiver and waiver services for high profile or crisis cases.
- Participate in the Greer Discharge Committee with appropriate recommendation for community placement for service recipients.
