Annual Salary:
Level I: - Y15A - $58,055.13 + Full State Employee Benefits
Level II: - Y15B - $62,409.27 + Full State Employee Benefits
Travel is occasional - Must possess a valid driver's license and must maintain required car insurance.
Minimum Qualifications:
Level I:
- Possession of a valid permanent Oklahoma license as a registered nurse, as approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC)
- Two years of professional nursing experience
Level II:
- Possession of a valid permanent Oklahoma license as a registered nurse, as approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC)
- Three years of professional nursing experience
Job Responsibilities
- The Residential Transitions HCMN II is responsible for supporting recipients as they transition from non-residential services to residential services within DDS.
- Key responsibilities include: obtaining, reviewing, and analyzing medical and health information for recipients transitioning into Residential DDS services.
- Complete initial Physical Status Reviews (PSRs) to assess the health status and care needs of incoming residential recipients.
- Provide consultation on health-related issues for recipients currently receiving non-residential services.
- Review health data and information through phone, digital media, and in-person visits as necessary to determine whether community-based residential services can adequately meet the recipient’s needs.
- Assist in identifying and coordinating professional and medical supports in the community to ensure continuity of care for residential recipients.
- Attend Housemate/Roommate Meetings for recipients entering DDS Residential Services to provide health guidance and support.
- Manage a small caseload of Health Care Level 1 and 2 recipients, providing oversight and consultation as needed.
- Collaborate with interdisciplinary teams, service coordinators, and family members to facilitate safe and effective transitions into residential settings.
KNOWLEDGE, SKILLS, AND ABILITIES (KSA's)
- KSAs required include: the knowledge, skills, and ability to analyze complex health concerns and issues, including the evaluation of adherence to established health care standards and the effectiveness of health management plans and/or programs, and recommending changes or revisions.
- Employees have a high degree of independence in planning and completing work assignments and may function as the lead person on some special projects. Also requires professional nursing knowledge in reviewing, auditing, and analyzing nursing and healthcare services provided by direct care providers to ensure that the appropriate healthcare resources are used at the appropriate level to meet the healthcare needs of clients.
