Case Manager IV - H21D
Annual Salary: $53,213.65 + Full State Employee Benefits
Travel is EXTENSIVE. Must possess a valid driver's license and must maintain required car insurance.
Minimum Qualifications
- A Bachelor’s degree in a human service field* and four years of professional experience working with individuals with intellectual and/or developmental disabilities in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, program coordination, nursing or a closely related field.
- OR a Bachelor’s degree and four years of professional experience in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, program coordination, nursing or a closely related field and one year of experience working with individuals with intellectual and/or developmental disabilities.
- OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and four years of professional nursing experience working directly with individuals with intellectual and/or developmental disabilities.
*For purposes of the Case Manager job family “a Bachelor’s degree in a human services field includes any degree from an accredited college or university except for a degree in a physical, natural or biological science or mathematics.
Job Responsibilities
This position is the supervisory position for professional field staff. It is responsible for theimplementation of program and policy changes within the unit as well as the resolution of problems at the local level. It is one of several positions of the same classification that are supervised by the field manager.
- Provides technical assistance and guidance to subordinate staff to ensure health and safety of service recipients, to resolve identified problems, to effectively communicate policy and procedure changes, interpret policy and practice to a wide variety of stakeholders, and ensure appropriate and complete documentation of targeted case management services.
- Provides technical assistance, support, guidance, and supervision for eight professional field staff; ensures adherence to policy, standard operating procedures, consent decree when applicable, all federal and state laws and guidelines.
- Monitor documentation to include but not limited to contact notes, Individual Plan documents, service plans, documents to support service justification, etc., to ensure the needs of the service recipients are addressed; address inadequate, incomplete, or missing documentation.
- Complete step 2 of the problem resolution process; intervene with providers and others as needed to ensure appropriate implementation of plans.
- Conducts and participates in staff training and meetings.
- Completes auditing tool and provides leadership coaching to staff.
- Works with others (DDS staff, other DHS staff, staff from other state agencies, other stakeholders as needed)to ensure proper linkage with DDS.
- Ensures completion of training requirements by subordinate staff as outlined in the Case Management Competency Checklist and policy. Completes performance evaluations.
